Immunization Requirements
As future health care providers, it is essential that you protect yourself and the community. Immunization standards and associated health practices are mandated by:

The Public Hospitals Act 1990, Revised Statutes of Ontario, Regulation 965 and any future legislated changes to the Public Hospitals Act

Communicable Disease Surveillance Protocols published jointly by the Ontario Hospital Association and the Ontario Medical Association and approved by the Ministry of Health
Download the Immunization Requirements Chart for an outline of steps and documents you are required to provide Health Services as proof of immunity for:
1. Tuberculosis
2. Hepatitis B
3. Measles
4. Mumps
5. Rubella (German Measles)
6. Varicella (Chicken Pox)
7. Tetanus/Diptheria
8. Pertussis
9. Polio

Please specify your program and graduating year on all copies of completed immunizations. A $10 processing fee will be charged to reproduce this information.
