Health Services collaborates with Michener clinical partners to ensure that students have their immunization records completed and up to date. When completing the Health Record Form, students must obtain and fulfill their specific immunization requirements from their family doctor or walk-in clinic. You may find your health record form in your admissions package, or download the health record form.

Clinical partners have the right to refuse students who do not meet the health requirements.
KEEP ORIGINALS for your professional portfolio and submit copies only. A $10 processing fee will be charged to reproduce this information. Please specify your program and graduating year on all copies of completed immunizations.
